A pediatric self-management program, You’re in Charge! was designed to prepare youth (13 to 15 years old) to initiate self-management of their chronic condition, and to enable their parents to initiate discussion and negotiation of behaviors leading to more independent self-management of the chronic condition.
This evidence-based intervention was developed by the IWK Health Centre, the Dalhousie School of Occupational Therapy, Chronic Condition Management Research Group, and by youth who live with chronic health conditions. The program is led by a team of trained youth leaders and parent/professional facilitators with support from the Coordinator. You’re in Charge evaluations are seeing excellent outcomes for both parent and youth participants.
You’re in Charge! Education, adventure and art-based programming has been designed for teens with chronic conditions, and their parents. Through activities, games, art, and group discussions, teens will create their own health summary, identify symptoms they need to be aware of, explore managing their medication, create their own personal action plan and set a goal for themselves. Parents will have a chance to connect with other parents and learn about helping their teen take on more responsibility.
One parent is required to attend. There is no fee to attend.
